These Terms of Service (“Terms”) outline the agreement between you (“Client,” “you,” “your”) and Elevate Mobile IV (“we,” “our,” “us”) regarding the use of our mobile IV therapy services. By booking or receiving services from us, you agree to these Terms.
Services Provided
Elevate Mobile IV provides mobile intravenous (IV) hydration and vitamin therapy services at your chosen location within our West Virginia service area. All treatments are administered by licensed and qualified medical professionals.
Medical Disclaimer
- Our services are intended for wellness support and are not a substitute for professional medical advice, diagnosis, or treatment.
- You should consult your primary care provider before receiving IV therapy, especially if you have any underlying medical conditions.
- In an emergency, call 911 immediately — we are not an emergency medical provider.
Booking & Payment
- Appointments may be booked online, by phone, text, or email.
- Payment is processed through Square at or before the time of service.
- All prices are subject to change without notice.
Cancellation & Rescheduling
- Please provide at least 24 hours’ notice to cancel or reschedule your appointment.
- Late cancellations or no-shows may result in a cancellation fee.
Eligibility for Services
- You must be 18 years or older to receive services.
- Clients under 18 must have a parent or legal guardian present to provide consent.
- We reserve the right to decline service to anyone if, in our professional judgment, treatment is not appropriate or safe.
Risks & Client Responsibility
By booking our services, you acknowledge:
- All medical treatments, including IV therapy, carry potential risks such as bruising, discomfort, infection, or allergic reaction.
- You have disclosed all relevant medical history to our staff prior to treatment.
- You release Elevate Mobile IV and its employees from liability for any adverse effects, except in cases of gross negligence or willful misconduct.

Limitation of Liability
To the fullest extent permitted by law, our total liability for any claim related to your use of our services will not exceed the amount you paid for that service.
Governing Law
These Terms are governed by the laws of the State of West Virginia. Any disputes will be resolved in the appropriate courts located in West Virginia.
Changes to These Terms
We may revise these Terms at any time. Updated versions will be posted on our website, and continued use of our services constitutes acceptance of the updated Terms.
